All nominations for the Gujarat SMILE Awards shall be accepted only through the official online nomination process hosted on the awards website

- Category-wise Google Forms
Separate Google Forms have been created for each individual award category.
Applicants must carefully select the relevant category and submit nominations only through the designated form linked to that category.
- Access to Nomination Forms
The nomination forms can be accessed by clicking the “Nomination Form” button placed alongside each award category on the website:

- One Form per Category
Each Google Form corresponds to one specific award category only.
Companies wishing to apply for multiple categories must submit separate nominations through the respective category-wise forms.
- Mandatory Self-Nomination
Only self-nominations submitted through the official Google Forms will be considered valid.
Recommendations or market-based recognition without formal nomination submission will not be considered for evaluation.
- Complete & Accurate Information
Applicants must ensure that all mandatory fields in the Google Form are duly filled, and relevant supporting documents are uploaded wherever required.
Incomplete, inaccurate, or partially filled forms are liable to be rejected without further consideration.
- Performance Period & Regional Scope
Data submitted through the Google Forms must strictly relate to:
Performance Year: FY 2024–25 only
Geographical Scope: Operations within Gujarat Region only
- Submission Deadline
Google Forms will remain open only until the announced nomination deadline.
Submissions received after closure of the forms will not be accepted, and no extensions shall be entertained.
- Acknowledgement of Submission
Upon successful submission of the Google Form, an auto-generated confirmation email will be sent to the registered email ID of the applicant, serving as acknowledgement of receipt.
- Verification & Jury Evaluation
All information submitted through the Google Forms is subject to verification by the organisers and evaluation by an independent Jury Panel.
The Jury reserves the right to seek additional information or clarification, if required.
- Final Authority
Submission of a nomination through the Google Form does not guarantee selection or award.
The decision of the Jury Panel shall be final and binding, and no correspondence shall be entertained in this regard.
- Responsibility of Applicant
It is the sole responsibility of the applicant organisation to ensure timely and correct submission of nominations through the official Google Forms.
The organisers shall not be responsible for non-submission, technical delays, or incorrect category selection by the applicant.
